Nursing Home Inspection Reports and Form CMS-2567

What each home's inspection reports found is summed up in these files. The reports themselves use Form CMS-2567, also called the Statement of Deficiencies and Plan of Correction. Every listed problem names the F-tag number for the federal rule that was not met. It adds a letter for scope and severity running A to L, what inspectors saw, and the fix planned by the home.

Open the home's profile on Medicare's Care Compare; its own reports sit in the inspections section. Asking the home is another option. Federal rules require the home to post its most recent inspection results where residents and families can see them. It must also show any inspection and complaint reports covering the past three years when asked.

One row in the citation file equals one finding from such a report. It gives the home's CMS number, state, inspection date, F-tag, and severity letter. It also notes whether it began with a complaint visit and when the home said correction happened.
