High Abuse Citations Across Arizona Nursing Homes
The federal freedom from abuse and neglect rule drew 303 citations from Arizona nursing home inspectors during the last three inspection cycles. That made it the most common violation across the state's 140 homes. The figures come from the federal nursing home records that CMS, the federal Medicare agency, publishes, and the same files show 40 Arizona facilities carrying a federal abuse flag. Inspectors wrote 3,009 citations statewide during the three cycles. In 77 of them, they found actual harm to a resident or immediate danger to a resident's health.
Since September 2023, federal regulators have imposed fines totaling $1,249,958 on 38 Arizona facilities. A $134,892 fine issued in May 2024 to Rehab at Scottsdale Village Square in Scottsdale was the largest single federal penalty, and that home is also the state's 1 Special Focus Facility, a status tied to a history of serious problems there. According to staffing reports, an average of 45.1% of nursing staff left Arizona homes in a year, against 45.8% nationwide; overall stars averaged 3.3, next to 3.0 for the nation as a whole.

Arizona Nursing Home Inspection Reports
A searchable state database, AZ Care Check, lets families review enforcement actions, licensing histories, and deficiencies. It is operated by the Division of Licensing at the Arizona Department of Health Services. On Medicare Care Compare, once Nursing Home Compare, every Arizona facility page has a direct link for federal inspection findings. It opens Form CMS-2567, the deficiency statement describing what inspectors saw during visits.
For unsafe care in an Arizona nursing home, anyone can file a report at 602-364-2536. The Arizona Department of Health Services also accepts reports through its online health care complaint page. For an advocate to help work out care problems, residents and families can reach the Office of Arizona State Long-Term Care Ombudsman Program at (602) 542-6454 extension 9; the office is housed within the Arizona Department of Economic Security, Division of Aging and Adult Services. If a vulnerable adult may be suffering abuse, neglect, or exploitation, that too can be reported to Arizona Adult Protective Services by calling 1-877-SOS-ADULT (1-877-767-2385).

Speaking With an Arizona Nursing Home Abuse Lawyer
Call 911 first whenever an Arizona resident faces immediate danger right now. Civil claims follow the general Arizona injury filing deadline: 2 years, as set out in Ariz. Rev. Stat. §§ 12-541 et seq.; a nursing home or medical claim may have a different deadline, and sometimes a shorter one. A paid or court-appointed caregiver commits a felony under Ariz. Rev. Stat. § 46-455 if neglect places a vulnerable adult in danger. After neglect, abuse, or exploitation causes harm, that adult may bring a superior court case against the individual or business hired to give care. The family can have a lawyer check the dates.
The provider information and health deficiencies files from CMS Care Compare supply the data.
