Low Nurse Staffing Across Indiana Nursing Homes
In Indiana, 402 of 505 nursing homes that reported data showed daily nurse hours under the level CMS, the federal Medicare agency, expects in its published facility records. In the state, residents get 3.69 nurse staff hours per day on average, below the national level of 3.86 hours. In these facilities, 45.9% of nursing staff left within a year; the nationwide share is 45.8%. In the state, corporate chains operate 425 of the homes, and American Senior Communities, with 90 locations, is the largest chain.
Supervision and accident hazards drew 606 citations from inspectors during the last three inspection cycles; 133 cited harm to residents or an immediate threat. Among federal standards, that rule leads the state in harm-level citations. The Special Focus program, which covers facilities with ongoing inspection problems, also includes Brookside Care Strategies, Core of Dale, and Munster Med-Inn under federal tracking. Regulators handed Valley View Healthcare Center in Elkhart the state's largest single federal fine, $284,560, in February 2026.

Indiana Nursing Home Inspection Reports
State inspection records are published by the Indiana Department of Health in the Health Facilities Consumer Reports map lookup. Every nursing home statewide has a consumer report on that tool. It summarizes survey findings, gives five years of federal, complaint, and licensure survey history, and links to each full report. Every one of the state's 507 homes has a page on Medicare's Care Compare, a federal search site formerly named Nursing Home Compare. The page links to the home's Form CMS-2567 deficiency statement.
To report concerns about resident care at a local facility, phone the Indiana Department of Health at 1-800-246-8909. Another option is the agency's online complaint page. The Indiana Long-Term Care Ombudsman Program is based within the Indiana Family and Social Services Administration. Families seeking a resident advocate can call 800-622-4484 or use the ombudsman complaint page. Another option is to report suspected neglect, battery, or exploitation involving an endangered adult in Indiana to Adult Protective Services by calling 800-992-6978.

Speaking With an Indiana Nursing Home Abuse Lawyer
When an Indiana nursing home resident faces danger right now, 911 is the first call to make. Under the chapter on adult protective services at Ind. Code 12-10-3 makes Indiana a state where reporting is mandatory, the Family and Social Services Administration says: any person who suspects battery, exploitation, or neglect involving an endangered adult must tell Adult Protective Services or law enforcement. Indiana sets the general injury filing deadline at 2 years, per Ind. Code §§ 34-11-2-1 and the sections that follow, though different, sometimes shorter deadlines can apply to nursing home or medical claims. The exact dates are something an Indiana attorney can check for the family.
The provider information and health deficiencies files from CMS Care Compare supply the data.
