High Staff Turnover and Harm Citations in Wyoming

Nursing staff turnover in Wyoming averages 51.8% a year, against 45.8% nationwide. Only six states have a higher rate. All 36 Wyoming homes appear in the public records on nursing homes kept by CMS, the federal Medicare agency. Someone filing a complaint with state regulators, rather than a routine yearly check, triggered 45.3% of all inspection citations statewide.

Over the last three inspection cycles, Wyoming facilities received 61 citations for infection control failures. Under the rule that keeps a resident safe from abuse and neglect, inspectors also wrote 51 citations in Wyoming. Of those abuse citations, inspectors found real harm to a resident or an immediate danger in 33. Regulators fined the Wyoming Retirement Center in Basin $105,284 in October 2025, the state's largest single federal penalty since September 2023. Wyoming's only Special Focus Facility home is Legacy Living and Rehabilitation Center; the program adds federal inspections for troubled facilities.

Wyoming Nursing Home Inspection Reports

The Wyoming Department of Health, Healthcare Licensing and Surveys posts state inspection findings on its Newest Facility Inspection Reports page, where families can read them. Dated PDF reports on revisits, complaint surveys, and state surveys appear in this alphabetical directory of state facilities. Federal reports for each Wyoming home can also be opened from its page on Medicare Care Compare, a search tool once called Nursing Home Compare. In those federal files, the Form CMS-2567 statement of deficiencies records what inspectors found wrong at all 36 homes in the state.

When care goes wrong, phone the Wyoming Department of Health, Healthcare Licensing and Surveys at 307-777-7123, or submit its online complaint form. A free resident advocate is also available to families through the Wyoming Long Term Care Ombudsman Program, reachable at 307-287-7757. Wyoming Senior Citizens, Inc lists regional advocates for the ombudsman office, which the state health department hosts. The state Department of Family Services Adult Protection Services can also receive reports about abuse or neglect of a vulnerable adult.

Speaking With a Wyoming Nursing Home Abuse Lawyer

Call 911 first when a Wyoming nursing home resident is in immediate danger. According to Wyoming law, Wyo. Wyo. Stat. § 35-20-103 says anyone who reasonably suspects a vulnerable adult is being neglected, abused, abandoned, intimidated, or exploited must act at once. The report must go to the Department of Family Services or law enforcement. For a state facility, such a report covers both the resident and how adequate the facility environment is. Under Wyo., this state sets a general deadline for injury claims of 4 years. Stat. §§ 1-3-102 et seq., though a nursing home or medical claim may be subject to a different, sometimes shorter deadline. A lawyer can verify the Wyoming deadlines for the family.
