Illinois Ranks First in Harm Citations and Fines
Out of the 50 states and the District of Columbia, Illinois ranks first for federal fines per home as well as per-home citations for harm to residents or immediate danger. CMS is the federal Medicare agency, and it has released inspection records. They show inspectors logging 25,359 total violations across 666 nursing homes statewide during the past three inspection cycles. Inspectors found actual harm to residents or immediate danger in 3,032 of those violations. That puts the state far above the national average of 1.54 citations per facility involving harm or danger. In addition, the state's average overall rating is 2.5 stars, versus a national average of 3.0.
State inspectors cite accident hazards and supervision under rule F689 more than any other violation, with 2,349 total citations, including 1,062 that involved resident harm. Federal officials have imposed penalties totaling $73,934,986 at 458 of the state's homes since September 2023. The largest single fine, $448,601, was issued in June 2025 to La Bella of Cahokia. Reported staffing at state facilities averages 3.45 hours of nursing care per resident day, under the 3.86 national average. The reported one-year share of nursing staff who left is 44.5%.

Illinois Nursing Home Inspection Reports
State disciplinary actions appear in the Quarterly Reports of Nursing Home Violators, where families can see which facilities the Illinois Department of Public Health lists for enforcement under the Nursing Home Care Act. On Medicare Care Compare (once called Nursing Home Compare), the page for any Illinois facility leads directly to complete federal inspection histories through Form CMS-2567, which lists that home's full statements of deficiencies. Of all citations in Illinois, 43.9% arise from complaint investigations instead of routine inspections.
To report unsafe care at an Illinois facility, anyone can file a formal complaint with the Illinois Department of Public Health by calling 1-800-252-4343 or through the state health care complaint page. For help from an advocate with day-to-day care problems, residents and families can contact the Illinois Long-Term Care Ombudsman Program at the Illinois Department on Aging through the Senior HelpLine at 1-800-252-8966. Reports of suspected abuse, neglect, or financial exploitation may also go to Illinois Adult Protective Services at 1-866-800-1409.

Speaking With an Illinois Nursing Home Abuse Lawyer
When an Illinois resident faces immediate danger right now, make 911 the first call. Under 735 Ill. Comp. Stat. 5/13-201 et seq., Illinois allows 2 years for a general personal injury filing, though a claim involving a nursing home or medical care can come with a different, sometimes shorter deadline. 210 ILCS 45/3-602 (Nursing Home Care Act) requires the licensee to pay actual damages, attorney's fees, and costs after a violation of a resident's rights under the Act. Section 3-601 also makes owners and licensees liable when intentional or negligent staff acts injure a resident. A family can have an Illinois lawyer verify the dates.
The provider information and health deficiencies files from CMS Care Compare supply the data.
