New Hampshire Homes Rank Lowest for Harm Citations

New Hampshire places 51st for citations per nursing home where inspectors found harm or immediate danger, across the 50 states and Washington, D.C. In the last three inspection cycles, inspectors cited 23 violations for harm to residents or immediate danger in the state's 73 homes. That is well below the U.S. rate of 1.54 serious citations per home. These state and national figures all come from the inspection files for nursing homes that CMS, the federal Medicare agency, publishes. In New Hampshire, the rule inspectors cited most often covers infection control and prevention, with 100 citations statewide.

Since September 2023, federal regulators have also fined New Hampshire nursing homes $578,772 in total. A $72,924 federal fine, the largest single penalty in the state, went to Lebanon Center, Genesis Healthcare in August 2025. The facility sits in Lebanon, and the state's largest nursing home chain runs it. In New Hampshire, staffing records put the average share of nursing staff leaving in a year at 44.1%, slightly under the national rate of 45.8%.

New Hampshire Nursing Home Inspection Reports

Nursing home inspection reports cannot be searched online through New Hampshire's state health officials. Their portal lists only professional licenses and pages for reporting incidents at facilities. Families can instead open Medicare's Care Compare website, formerly named Nursing Home Compare, to see federal reports on inspections at any New Hampshire home. On that site, every New Hampshire facility page shows Form CMS-2567, the official deficiency statement with inspectors' notes from routine visits and complaint investigations.

People concerned about resident care can submit a health facility complaint to the New Hampshire Department of Health and Human Services, Health Facilities Administration, or phone the agency at 603-271-9049. To settle a care issue with an advocate's help, residents and families can dial (800) 442-5640 for New Hampshire's Office of the Long-Term Care Ombudsman. If a vulnerable adult in New Hampshire may be facing neglect, abuse, or exploitation, phone 1-800-949-0470 for the state's Adult Protective Services office.

Speaking With a New Hampshire Nursing Home Abuse Lawyer

Call 911 first when a nursing home resident in New Hampshire faces danger right now. The New Hampshire Patients' Bill of Rights, N.H. Revised Statutes Ann. § 151:21 requires each licensed facility's policy to cover freedom from abuse, neglect, exploitation, chemical or physical restraints, and involuntary seclusion, unless a physician authorizes restraints in writing. In New Hampshire, the general personal injury filing deadline is 3 years under N.H. Rev. Statutes §§ 508:1 et seq.; still, a nursing home or medical claim may carry a different, sometimes shorter deadline. The family can have a New Hampshire attorney check the dates.
